Among the many sectors that it transforms, the<strong>health industry<\/strong> is no exception to the rule.<\/p><p><strong><a href=\"https:\/\/www.kapcode.fr\/en\/\">Kap Code<\/a> <\/strong>offers you to discover through <strong>several articles the crossed views of three specialists in digital health<\/strong>. Deceive through their visions this transformation of the world of health, through digital and data-science.<\/p><p>In this <strong>third and last part<\/strong>the three speakers share their vision on obstacles concerning data science in healthcare today, both for you and more globally?<\/strong><\/h2><p><strong>IH:<\/strong> \"Healthcare is a sector that is traditionally very protective and that's fine by the way! <strong>When you are a health actor, you have to make sure you comply with the regulations<\/strong>It is important to be careful with the framework of relations with patients and health professionals. These are many <strong>necessary precautions but which can also lead to slowness in the progress of projects and limitations<\/strong>.<\/p><p><strong>Tech players ask themselves far fewer questions and take \"risks<\/strong>. As a result, projects move more quickly and more easily outside the framework. A gap can therefore be created in the progress of joint health and tech projects.<\/p><p>Most of the health actors still have few skills in everything that will allow the use of digital and data, but many recruitments of \"new\" profiles are underway and in parallel we see <strong>a real strategic reflection<\/strong> on how to <strong>the use of data can contribute to the missions of the different health actors<\/strong>. The healthcare ecosystem still lacks the maturity to implement strategic data projects but <strong>there is an appetite among healthcare players and also data scientists that has accelerated significantly in recent months<\/strong>.<\/p><p><strong>Healthcare is a data-intensive sector<\/strong>which is meaningful - which is very important for data scientists - and which has significant resources, as well as<strong>elements that will make it one of the leading fields of data science in the years to come<\/strong>.<\/p><p>Many projects are being initiated, with <strong>accelerated digitization of the healthcare ecosystem, reinforced by the period of crisis management of COVID<\/strong>. They aim to improve the care pathway, develop research, invent new services, better understand the needs of patients and health professionals...<\/p><p>To illustrate with a project in which I am involved, the<a href=\"https:\/\/www.aphp.fr\/\" target=\"_blank\" rel=\"noopener\">AP-HP\u00a0<\/a>and in particular the team in charge of the Health Data Warehouse, in partnership with <a href=\"https:\/\/www.datacraft.paris\/\" target=\"_blank\" rel=\"noopener\">datacraft<\/a> and the <a href=\"https:\/\/scai.sorbonne-universite.fr\/\" target=\"_blank\" rel=\"noopener\">Sorbonne Center for Artificial Intelligence<\/a>The aim of the project is to encourage the widest possible community to collaborate on health issues and to promote the emergence of multidisciplinary collaborative projects and the exchange of best practices on subjects such as NLP, computer vision and data FAIRification applied to health.<\/p><p>One of the main challenges of using data is <strong>the responsibility associated with it<\/strong>. There is a lot of talk about the reliability and bias of algorithms. It is necessary both that <strong>everyone is aware of this and becomes an actor in the use of data and the values they wish to associate with it<\/strong>. For a company, for example, recruiting a data scientist is not only the headache of ensuring his skills and knowing how to integrate him into the organization, but it is also ensuring that he shares the company's values because this can be less easy to measure afterwards for someone who is going to develop algorithms than for a legal or marketing director!<\/p><p>There is a real<strong> acculturation to be done to the use of data and its responsible and sustainable use<\/strong>. Today, in all schools that train data scientists, there is an awareness of this subject. <a href=\"https:\/\/www.datacraft.paris\/\" target=\"_blank\" rel=\"noopener\">datacraft<\/a> has been created in particular with <strong>the mission to allow data scientists to think about why they do things, how they do them, what limits they set themselves<\/strong>. For example, several of our members have decided to write a charter on the proper use of data together. There is a real value in doing this in a collaborative way, by allowing cross-sector and cross-organizational views.<\/p><p>It is a complicated subject, if we compare France with China or the USA, we do not have the same visions, just look at the ethical debates on the decisions to be taken in the programming of an artificial intelligence that would have to decide to avoid one person rather than another in case of an accident. There is really an exchange to be had, an assumption of responsibility at the international level to be developed. <strong>I don't think that ethics can be a solely national vision.<\/strong> \"<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-89c5d3a elementor-widget elementor-widget-image\" data-id=\"89c5d3a\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"image.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t\t\t\t<figure class=\"wp-caption\">\n\t\t\t\t\t\t\t\t\t\t<img decoding=\"async\" src=\"https:\/\/www.kapcode.fr\/wp-content\/uploads\/elementor\/thumbs\/Isabelle-Hilali-pb10qutm6jf9exzkj0m0yijudgb5c8u46s9kdf8swu.jpg\" title=\"Isabelle Hilali\" alt=\"Isabelle Hilali - Founder &amp; CEO of datacraft\" loading=\"lazy\" \/>\t\t\t\t\t\t\t\t\t\t\t<figcaption class=\"widget-image-caption wp-caption-text\">Isabelle Hilali - Founder &amp; CEO of datacraft<\/figcaption>\n\t\t\t\t\t\t\t\t\t\t<\/figure>\n\t\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-d339e55 elementor-widget elementor-widget-text-editor\" data-id=\"d339e55\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p><strong>PF:<\/strong> \"To frame my answer, I'll talk about what I know best: <strong>opportunities and obstacles<\/strong> that we encounter internally at Kap Code with our tool <a href=\"https:\/\/www.kapcode.fr\/en\/medical\/\">Detec't<\/a>.<\/p><p>First, we can identify problems with the data itself:<\/p><ul><li>Is the data of good quality?<\/li><li>How to find coherent and interesting information in the mass?<\/li><\/ul><p>As our data comes from social networks, we also have to learn how to work with very poorly formatted text (length, mistakes, specific language), and we don't have large pre-annotated databases, which requires a lot of time to create them and train our algorithms afterwards. It is therefore <strong>A first obstacle is the availability and quality of the data.<\/strong><\/p><p><strong>Another difficulty encountered is at the level of recruitment.<\/strong> Indeed, the <strong>data-science being a new profession<\/strong> it was difficult to find candidates with a background in NLP (Natural Language Processing). <strong>Few profiles are trained in this technology and other companies in the market are recruiting.<\/strong> I myself did not have enough experience to judge the competence of others. Nevertheless, today we are starting to see more and more NLP oriented trainings, <strong>Many schools are launching this type of training,<\/strong> but there is still a lack of autonomous and qualified profiles.<\/p><p>We also have <strong>challenges specific to R&amp;D:<\/strong> In case of problems, we can rarely refer to existing works.  We rely on bibliography, forums, but often we don't know if our projects will work, we try different methods, and then we adjust according to the results. There have been times when we realized that one of our goals was unattainable.<\/p><p>Finally, <strong>at the technical level,<\/strong> we have models that take a long time to run, technologies that are not yet optimized internally and that can make us lose time and slow us down. <strong>We are getting better every day by getting more organized, but we still have a long way to go.<\/strong>\u00a0\"<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-7e2986e elementor-widget elementor-widget-image\" data-id=\"7e2986e\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"image.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t\t\t\t<figure class=\"wp-caption\">\n\t\t\t\t\t\t\t\t\t\t<img decoding=\"async\" src=\"https:\/\/www.kapcode.fr\/wp-content\/uploads\/elementor\/thumbs\/Pierre-Foulquie-pb10s9kwflcqtvxuamjvp5qqg9cyvwfmdrhsad5jku.jpg\" title=\"Pierre Foulqui\u00e9\" alt=\"Pierre Foulqui\u00e9 - Data Science Manager at Kap Code\" loading=\"lazy\" \/>\t\t\t\t\t\t\t\t\t\t\t<figcaption class=\"widget-image-caption wp-caption-text\">Pierre Foulqui\u00e9 - Data Science Manager at Kap Code<\/figcaption>\n\t\t\t\t\t\t\t\t\t\t<\/figure>\n\t\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-b7c0536 elementor-widget elementor-widget-text-editor\" data-id=\"b7c0536\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p><strong>EC:<\/strong> \"For more than 15 years in the pharmaceutical industry and working on these subjects for almost 10 years, I have observed and experienced many evolutions and stages of maturity of our French ecosystem. <strong>The challenges are protean, numerous and constantly evolving, especially on the subject of AI in health.<\/strong><\/p><p>Today, the strongest are certainly <strong>Governance<\/strong>,<strong> access to the market<\/strong> and <strong>permanent integration<\/strong> in the uses and measurement of its impact\/value.<\/p><p>Let's start with governance: This is the first aspect: who is in charge? Who decides? Who drives the strategic choices? How do we all agree together? When we see <strong>the complexity of governance<\/strong> in some digital initiatives we realize that so many people are involved in deciding that <strong>decision times are very long.<\/strong><\/p><p>Since digital technology is very fast, if the decision processes are slow, we lose competitiveness, we even create a loss of chance for the patient: <strong>we can't bring the solution to market simply because we need to get 5, 10, 15 or 20 players to agree together<\/strong>.<\/p><p>Moreover, in the notion of governance <strong>the perception of the value and assets of each can be quite variable.<\/strong> Everyone wants to keep the value they have, so we can reach major sticking points when we have to make decisions together.<\/p><p>Second point, <strong>access to the market<\/strong>\u00a0<b>:<\/b> If we want to implement and deploy health solutions, technological and digital devices in an equitable and secure manner, then these are <strong>Medical Devices<\/strong> <b>(DM).<\/b> However, today, many players say to themselves \"well, I've done my thing, I've tested it, it works: I can go! But in fact, they have no level of maturity in terms of regulations, certification <strong>DM<\/strong>registration files.<\/p><p>There are many players, not necessarily start-ups, but also medium-sized or even large structures that, when told that their solution will be a <strong>DM<\/strong>The answer is \"not at all\". But any specialist will say yes, it is a DM.<\/p><p>They want to avoid all the notions of quality, development, IT security that allow sufficient specifications to access the market, while these are important issues but require sufficient maturity of the product.<\/p><p>And beyond the level of maturity there is also <strong>the will.<\/strong> There are players in digital health who have seen how much being <strong>a DM was going to increase their delay on their <i>roadmap<\/i> and cost more.<\/strong> So they try to avoid that, they're in <strong>circumvention strategies,<\/strong> which means that they will launch their solution faster and cheaper but run the risk of being caught by the European or French authorities.<\/p><p>That is why<strong> the notion of market access and all that is regulatory and quality is still major.<\/strong> Maybe it will be less so in 5 years because the ecosystem will have progressed.<\/p><p>Third point: <strong>The perennial use in the uses.<\/strong> When you look at the number of mobile health applications on the store and when you see the ones that are used, there is an enormous loss rate. When you see the number of bracelets or activity trackers that are bought and those that are used after 3 or 6 months, it's catastrophic. <strong>Once innovations get past the issues of governance and market access, it is not enough for people to use them over time in a sustainable way.<\/strong><\/p><p>So <strong>In my opinion, the question must be asked on several levels:<\/strong> does the solution really meet a need? If it does not meet a need, will that need appear?<\/p><p>And if, on the other hand, we say to ourselves that there is a decision-making stakeholder, we also run the risk of breaking up because the other stakeholders get their value sucked out and will either die or disagree. That's why<strong> the notion of governance comes first for me,<\/strong> it's a good starting point. Today in a project, for example a data platform, the complexity is not the technology but the governance very clearly. And who implies governance implies <i><strong>business model<\/strong><\/i>. <strong>The <i>business model<\/i> is included in the notion of governance.<\/strong><\/p><p>If that's the case, so much the better, but in that case how do we deploy? How do we distribute? How do we sell? And how do we help people adopt it? These are all real challenges. It's a shame to see the number of companies that develop cool stuff, that deploy it after a lot of sweat and tears, only to find that after 6 months there is total indifference.<\/p><p><strong>Second aspect on integration in uses:<\/strong> when it comes to digital solutions, for example for hospitals, <strong>it can impact the organization of care,<\/strong> the definition of jobs, the scope of responsibilities. For example, in telemedicine solutions, which are regularly mentioned, the possibility of delegating procedures from the doctor to a nurse.<\/p><p><strong>When you put a digital solution in a hospital, you don't just make it available<\/strong> but also to say \"What does it change? How does it reorganize the care pathway? How does it reorganize the distribution of tasks and responsibilities of professionals?<\/p><p>Because if we don't make this effort, people will stay with their normal, existing organization and <strong>the solution will be perceived as \"extra\".<\/strong> Since they are not changing the organization, they will continue to operate as before and the solutions will not be used for more than a few months.<\/p><p>Finally, the fourth and last point:<strong> Impact and value<\/strong>. Among the many health technology solutions developed or deployed,<strong> How many are published in peer-reviewed journals to assess the value created in terms of health outcomes?<\/strong> Very little. And that's one thing that's missing.<\/p><p>What value does it bring to the patient? What value does it bring to the medico-economic point of view of the care system? What is the organizational value? What is the economic efficiency of a health care institution? This, <strong>you have to take the time to measure it,<\/strong> to formalize this measure and to follow it over time.<\/p><p>A bit like a drug! Many agree that the massification of digital and the development of AI is as profound a civilizational revolution as the literacy revolution. <strong>It is the opportunity for us and the few generations to come to define the desirable future.<\/strong><\/p><p>More pragmatically, <strong>the AI sector is in hyper-growth<\/strong> <strong>(+50% \/ year) in health<\/strong> with a size that is expected to reach more than 23 billion within two years.<\/p><p>As such, to keep control and exploit it positively,<strong> the subject of ethics must concentrate energies because this subject is still under considered (20% of the actors), not mastered and on too restricted fields<\/strong>\u00a0<b>:<\/b> Privacy and governance. This is very inadequate.<\/p><p>It is necessary to involve public and private actors in this ethical reflection and to extend the field:<\/p><ul><li>To individual fundamental rights vs. general interest<\/li><li>To autonomy and free will<\/li><li>To legal responsibility<\/li><\/ul><p><strong>We are still at the stage of very low artificial intelligence<\/strong> which leaves us the opportunity to work on major topics which, from my point of view, could be :<\/p><ul><li>The robustness of the solutions that will generate knowledge that will help in decision making and behavioral changes.<\/li><li>The comprehensible transparency of complex systems.<\/li><li>The non-discriminatory and inclusive nature of progress in relation to the social pact in France.<\/li><li>Liability in a broad sense and more specifically in a legal sense, for example with the definition of \"robot law\".<\/li><li>The transformation of professions, the in-depth reform of training and education.<\/li><li>Demonstrating value and assessing the positive impact on patients in the short, medium and long term<\/li><li>And finally the notion of governance...<\/li><\/ul><p>And I would like to return to the subject of <strong>Governance<\/strong> which is of primary importance to me. <strong>A clear and efficient governance is needed, driven by the general interest.<\/strong> It will be necessary to set up this structure in an adapted way thus on the same model as the Web developed: <strong>No centralization to address the issues nor delocalization but rather a distributed network betting on collective intelligence to create sustainable value.<\/strong><\/p><p>Indeed, if the governance model were centralized at the level of politicians and governments, for example, there would necessarily be an opposition effect from the technology actors.<\/p><p>Look at the tensions between GAFAM and governments. Each is trying not to go to the breaking point because each needs the other. And why did this happen? Because there has been an ultra-centralized system of governance. On both sides. If we are on a delocalized system, we will make the tensions diffuse, but there will always be tensions.<\/p><p>That's why I talk about <strong>distributed network,<\/strong> when I talk about a distributed network, it means that <strong>the share of voice of each in the governance is both balanced and diversified.<\/strong><\/p><p>When we have a functional solution that meets the needs, before taking the regulation in hand, it is necessary that <strong>the people who make these solutions have a strong sense of ethics and responsibility<\/strong> to themselves participate in the regulation of their solution rather than playing the pirate by saying \"there's a market opportunity, I'll go for it, and then they'll try to catch me, but in the meantime I'll have done some business. That's the distributed model: <strong>responsibility and ethics must be at the heart of our concerns<\/strong>not of some <i>stakeholders<\/i> but of everyone. To effectively address this perimeter that belongs to everyone and to no one, that has infinite value but is priceless ... <strong>Health!<\/strong>\u00a0\"<\/p><p><strong>PF:<\/strong> \"It is important to keep in mind that <strong>data science is above all based on data!<\/strong> And in the context of health, sensitive data, critical to the health of the patient. This is not a commercial site, <strong>the issues at stake will have a big impact on the patient,<\/strong> such as knowing whether a person has cancer or not. So there is a big notion of responsibility to take into account: if the program is wrong, whose fault is it? The programmer? The doctor?<\/p><p><strong>The real issue for me today is therefore at the ethical level<\/strong> and it's a fascinating subject. There's a lot of work to be done in terms of not perpetuating the junk and looking beyond the program.<\/p><p>Then there are obviously<strong> technical issues.<\/strong> From 1990 to 2010 they used programs based on the knowledge of the 40s, via the new powers of calculation. Today, we are really reaching innovations in terms of math, there is a lot of progress, even if the results for the moment only allow to recognize a banana on a picture (<em>laugh<\/em>).<\/p><p>And finally, if we focus on a more global and long-term vision, I would say that there is a <strong>societal issue.<\/strong> Health AIs seek to enable people to live longer and healthier lives. This is a fairly classic scenario, not only in the health sector, with <strong>the result is demotivated data scientists who don't stay<\/strong> and the rest of the organization that does not see the value of the projects implemented and the resources mobilized because no one sees results.<\/p><p><strong>On projects involving data, we need a real vision linked to our missions, but also \"quick wins\",<\/strong> of projects that unite and learn by doing.<\/p><p><strong>In health care we need to involve all actors including patients and health professionals<\/strong> and as in all other fields to create a link with data experts. In France, <strong>we need to accompany the projects,<\/strong> because there is <strong>access to data is more complicated than in some other countries.<\/strong> There are many startups and big players such as pharmaceutical companies that will use foreign data. It's a shame, when we can see the interest of having local data.<\/p><p>There is a real national effort being made with the <a href=\"https:\/\/www.health-data-hub.fr\/\" target=\"_blank\" rel=\"noopener\">Health Data Hub<\/a>but we are still at the beginning. There would be<strong> a real public policy to be carried out, with more means, which is what some countries do.<\/strong>\u00a0\"<\/p><p><strong>This last article<\/strong> presenting the crossed views of specialists in health innovation highlights<strong> Challenges and opportunities of Data Science in healthcare<\/strong>. This area is in <strong>evolving in a young ecosystem.<\/strong> Thus, <strong>the issues<\/strong> mainly echo the <strong>the role of the human being, his ethics and his responsibility<\/strong> in the creation and use of new models.
